About the Role

The Advanced Nurse Practitioner will be responsible for:

  • Leading the CHC team and providing expert care to patients with complex needs
  • Developing and implementing care plans that meet the individual needs of patients
  • Collaborating with other healthcare professionals to ensure seamless care delivery
  • Providing education and training to staff on CHC best practices
  • Ensuring compliance with CHC legislation and national guidelines
Requirements

To be successful in this role, you will need:

  • Registration as a Registered Nurse (RGN/RMN/RNLD/RSCN)
  • Postgraduate qualification or specialist training in Community Nursing
  • Minimum 2 years post-qualification experience as a Registered Nurse
  • Significant experience of both community and hospital working
  • Up-to-date knowledge of the national nursing agenda
