The role of the Frailty Practitioner - Elderly Care Specialist is to provide high-quality patient care and support to our elderly population. This involves working at the interface with Elderly Care consultants, developing the scope of nursing practice, and providing nursing excellence in patient care.
Main Responsibilities- Provide specialist and advanced clinical skills support to medical staff and the nursing team.
- Triage patients, prioritizing clinical need, highlighting and initiating treatment plans by following agreed Trust guidelines and protocols.
- Work autonomously assessing individual patient needs, initiating investigations, determining a differential diagnosis, and initiating appropriate holistic, evidence-based treatment and care.
- Coordinate clinical and support services while maintaining or improving the service.
- Role model compassionate and inclusive leadership in order to shape the creation of a collective leadership culture within the trust.

Person SpecificationQualifications
- 1st level registered nurse/AHP - with substantial experience
- Registered Mentor
- Degree level study to Masters level or equivalent experience
- Advanced clinical skills qualification at level 7
- Non-medical prescribing qualification
- Demonstrate thorough understanding of expanded scope of professional practice issues
- Evidence of continuing academic and professional development - ENB998/730 or equivalent.
Experience
- Appropriate clinical experience in a clinical setting
