We are seeking a highly skilled and experienced Clinical Specialist Frailty Occupational Therapist to join our team at University Hospitals Sussex NHS Foundation Trust. This is an exciting opportunity to work in a dynamic and supportive environment, providing expert care and assessment to patients with progressing life-limiting illnesses.
Main Responsibilities- Provide clinical excellence, leadership, and innovation in palliative care, working closely with multidisciplinary teams to deliver high-quality patient care.
- Undertake holistic assessments of patients' needs, ensuring that care is tailored to their individual requirements and communicated effectively to all relevant parties.
- Facilitate the discharge of patients in accordance with rapid discharge guidelines and their preferred place of care and death.
- Provide education and training to nursing and medical teams, promoting best practice in palliative care.
- Support junior members of the hospital palliative care team, fostering a culture of collaboration and knowledge sharing.
We are looking for a candidate with:
- NMC registration as an Adult Nurse.
- Specialist qualification in palliative care or a related field.
- Proven experience in palliative care, with a strong understanding of complex physical and psychological needs.
- Excellent communication and interpersonal skills, with the ability to work effectively in a team.
- Ability to use specialist assessment skills to plan, implement, and evaluate care.
