About the Role

We are seeking a highly skilled and experienced Primary Care Liaison Practitioner to join our team at South West London and St George's Mental Health NHS Trust. As a key member of our Primary Care Liaison Team, you will play a vital role in promoting clinical excellence and providing high-quality care to patients with mild to moderate mental health difficulties.

Key Responsibilities

  • Provide structured and focused liaison and consultation to other professionals, including GPs, Integrated Recovery Hubs, and voluntary organizations.
  • Organize and prioritize your own workload to ensure effective delivery of services to patients.
  • Develop and maintain positive working relationships with stakeholders, including service users, carers, and other professionals.
  • Ensure service users receive timely and effective Primary Care Mental Health interventions, as agreed by pathways, protocols, and NICE guidelines.
  • Process referrals and deliver comprehensive biopsychosocial assessments, diagnostic formulations, and clinical reviews.
  • Offer medication reviews and advice to patients, carers, and GPs.
  • Ensure effective transfer of secondary care service users to Primary care, including those on depot medication.
  • Support GPs and other professionals in primary care with assessing and treating patients through role modeling, facilitating, and training.
  • Deliver consultation, advice, and clinical supervision to other members of the clinical team regarding Primary Care service provision.
  • Lead multi-disciplinary and multi-agency clinical meetings when required.

Person Specification

Essential

  • Extensive post-qualification experience in a relevant field in Mental Health.
  • Substantial experience of working autonomously and independently in community settings.
  • Substantial experience in community caseload management and mental health liaison.
  • Experience of working with diverse agencies in a complex environment.
  • Experience of working with primary care.

Desirable

  • Experience of working with people with chronic physical health conditions.

Training & Qualifications

Essential

  • Recognized mental health qualification in nursing, social work, occupational therapy, or psychology.
  • Current Professional Registration.
  • Evidence of continual professional development.

Desirable

  • Qualification in a psychological treatment modality.
  • Approved to prescribe medication.

Knowledge & Skills

Essential

  • Ability to communicate effectively both verbally and in writing about a range of complex issues.
  • Highly developed problem-solving and conflict resolution skills.
  • Ability to use current IT systems and packages for data entry and analysis.
  • Ability to analyze and present data in a meaningful way.
