We are seeking a highly skilled and dedicated Community Practitioner to join our dynamic and flexible service providing unplanned and urgent care. As a key member of our team, you will play a vital role in supporting patients through acute episodes of ill health in their own homes, providing enhanced and complex care in a crisis, and handing over stable patients to Planned Care colleagues.

Key Responsibilities
  • Deliver compassionate person-centred care, treating patients, service users, their families, and carers with consideration, dignity, and respect.
  • Ensure safe and effective clinical practice, enhancing the patient's experience, and contributing to the delivery of the organisation's objectives.
  • Work harmoniously and effectively with colleagues, patients, and clients, seeking advice from senior practitioners as necessary.
  • Practise within the scope of personal professional competence, extending this scope as appropriate to service needs.
  • Delegate aspects of care to others, accepting responsibility and accountability for such delegation.
  • Participate in peer review of own practice, critically appraising and synthesising the outcomes of relevant research, evaluations, and audits.
  • Assess individuals holistically, using a range of different assessment methods, and incorporate health promotion and prevention into care plans.
  • Plan and co-ordinate complete episodes of care, delegating and referring to other practitioners as appropriate to optimise health outcomes and resource use.
  • Work within defined boundaries of practice, ensuring an effective and efficient patient's journey.
  • Develop a culture of person-centred care, promoting a caring and compassionate environment where equality and diversity issues are respected.
  • Contribute to the development of care and services by ascertaining patient and carer experience/feedback and ensuring complaints are managed in line with organisational policy.
