Job SummaryWe are seeking a compassionate and confident individual to join our Community Response Team as a Health Care Support Professional. The successful candidate will work alongside registered nurses and qualified therapists to deliver an admission avoidance service, providing clinical interventions and support to patients experiencing a health and social care crisis in their own homes.
Main Responsibilities- Visit patients in their homes to deliver a range of clinical interventions and support their recovery.
- Obtain informed consent and provide sensitive, high-quality patient care, encouraging patients and carers to take an active approach to achieving their goals and maintaining independence.
- Assist in identifying equipment and adaptations required in a patient's home and monitor patient progress, reporting back to registered staff.
- Complete accurate and timely clinical records in accordance with organisational, professional, and departmental standards and procedures.
- Maintain and develop knowledge, skills, and competence to ensure patients' individual needs are met in accordance with the band 3 competency framework.
- NVQ 3 or equivalent, including relevant experience.
- GCSE in English and Maths.
- Relevant experience in a similar role.
- Valid car driver's license and access to a car for work purposes.
