Key Responsibilities:

  • To provide a customized, responsive, and high-quality service to individuals with additional needs, supporting them in their own homes and facilitating access to learning and leisure opportunities.
  • To empower individuals to live as independently as possible, with appropriate levels of intervention and support, as outlined in their care plans.
  • To assist individuals in enhancing their communication skills, using constructive and total communication at all times, tailored to their needs.
  • To advocate on behalf of individuals, respecting their right to privacy, dignity, choice, and respect, while recording and reporting any safeguarding issues or concerns.
  • To maintain constructive contact with family and significant people involved in the care and well-being of individuals, while preserving professional boundaries and relationships.
  • To implement individuals' development or care plans, consistent with their culture, gender, and age, arranging and accompanying them on leisure activities and appointments.
  • To escort individuals outside of their homes, engaging in activities such as shopping, clubs, or other activities of their choice, and supporting them in making arrangements.
  • To be available for work 7 days a week, including night duties, unless otherwise agreed at the commencement of the post, and to comply with availability unless annual leave has been authorized.
  • To visit individuals in their homes, providing access to correspondence and written information, assisting with letter writing and making telephone calls, and acting as a companion to provide news and information.
  • To assist with personal care and medication, as stated in individuals' care plans and in accordance with their training and the clients' Support/Care Plan.
