We are seeking a highly experienced Social Worker to join our Transition Support Service team in Waltham Forest. As a key member of our team, you will work closely with young people with disabilities, their families, and other professionals to ensure a smooth transition from Children's to Adult Services.
Key Responsibilities:- Develop and implement transition plans for young people with disabilities, ensuring their needs are met and their potential is maximized.
- Work collaboratively with multi-agency teams, including health and social care professionals, to provide holistic support and care.
- Conduct statutory assessments, assess needs, and ensure eligibility for service provision.

- UK experience as a Social Worker, with a strong background in Learning Disability.
