Job Title: Social Worker - Transition Support Service

We are seeking a highly experienced Social Worker to join our Transition Support Service team in Waltham Forest. As a key member of our team, you will work closely with young people with disabilities, their families, and other professionals to ensure a smooth transition from Children's to Adult Services.

Key Responsibilities:
  • Develop and implement transition plans for young people with disabilities, ensuring their needs are met and their potential is maximized.
  • Work collaboratively with multi-agency teams, including health and social care professionals, to provide holistic support and care.
  • Conduct statutory assessments, assess needs, and ensure eligibility for service provision.
  • Coordinate assessments, share information, and lead on joint care planning to provide an understanding of needs and provision of services.
  • Ensure compliance with Safeguarding procedures and work within the duty system, leading on relevant investigations and meetings.
Requirements:
  • UK experience as a Social Worker, with a strong background in Learning Disability.
  • Registration with Social Work England.
  • Right to work in the UK.
