Position Overview

We are seeking a Band 6 Crisis Resolution Nurse to join our dedicated team.

The successful candidate will assist the Team Leader in the formulation and execution of a multi-disciplinary Crisis Resolution/Home Treatment Team, delivering exceptional care tailored to the needs of individuals experiencing acute mental health challenges and their families. This role is pivotal in ensuring the smooth operation of the team, providing guidance and oversight within the diverse staff group as necessary.

The role involves conducting urgent evaluations of individuals facing mental health crises and, when suitable, offering intensive community-based treatment and support services as an alternative to hospitalization. Our service operates continuously, ensuring availability around the clock throughout the year.

The individual will play a key role in establishing and maintaining effective team systems and processes, including the implementation of outcome measures and audits under the supervision of the Team Manager.

Main Responsibilities

Key Relationships:

1. Communication and Collaboration

Support the Team Leader in fostering effective communication both within the team and with external agencies.

Facilitate clear communication regarding referrals, admissions, and discharges from the Crisis Resolution/Home Treatment Team, ensuring seamless transitions between inpatient and community services.

Maintain productive partnerships with Sector Adult Community Mental Health Services, PATH, and other community-based teams, adapting to local needs.

Ensure that all pertinent information regarding client care is relayed to the appropriate parties, highlighting any concerns regarding practice or workload to the Team Manager.

Engage effectively with relatives, visitors, voluntary organizations, and the public, promoting their involvement in care delivery. Respond sensitively to the needs of families and caregivers, particularly in emotionally charged situations.

Collaborate with other professionals concerning client care, actively participating in care planning and review meetings, and attending case conferences as required.

Advise others on the roles and practices of the Crisis Resolution/Home Treatment Team.

Maintain positive working relationships and clear communication with team members and other professionals.

Job Responsibilities

1. Analytical and Decision-Making

Assess clients according to defined referral criteria in line with local operational policies.

Conduct risk assessments and management activities.

Plan, implement, and evaluate care programs for clients in crisis with acute mental health issues in the community, coordinating efforts with other professionals.

2. Planning and Organization

Allocate and adjust tasks or staff daily to meet organizational and workload demands, responding to urgent, unplanned client needs to ensure rapid assessment and intensive home treatment.

Assist the Team Leader in the effective daily management of the team, including duty rosters, time management, and annual leave, ensuring an appropriate skill mix at all times.

Act as a stand-in for the Team Leader as necessary.

Support the Team Leader in the recruitment, orientation, and induction of new staff.

Comply with relevant mental health and work-related legislation at all times.

Facilitate coordination, liaison, and integration of activities from other services or agencies in the provision of a Care Programme Approach (CPA) package.

Conduct service activities in accordance with the Crisis Resolution/Home Treatment Team Operational Policy.

Support the provision of a flexible and responsive service, initially from morning to evening, seven days a week.

Assist the Team Leader in the effective and timely management of complaints and serious adverse events as they arise.

Qualifications and Experience

Essential:

  • Registered Mental Health Nurse (RMN) or Diploma/BSc (Hons) in Mental Health Nursing.

Previous Experience

Essential:

  • Post-registration experience with some experience at Band 6.
  • Experience in acute and community settings.

Physical Requirements

Essential:

  • Ability to access buildings with stairs, including office or ward bases, and travel to various locations, including client homes.

Other Requirements

Essential:

  • Car driver (unless a disability as defined by the Equality Act 2010 prevents driving).
