About the Role

We are seeking a highly skilled Occupational Therapist to join our Specialist Palliative Care Team at Southern Health NHS Foundation Trust. As a key member of the team, you will be responsible for providing specialist care to patients with life-limiting illnesses, working closely with other healthcare professionals to ensure seamless care.

Key Responsibilities

  • Practice clinically across a variety of settings, including community and hospital environments.
  • Develop and support patient-centred care plans, working collaboratively with patients, families, and other healthcare professionals.
  • Contribute to the development of the service, participating in multidisciplinary team meetings and contributing to the design of care pathways.
  • Provide education and training to other healthcare professionals, promoting best practice in palliative care.
  • Participate in research and quality improvement initiatives, contributing to the advancement of palliative care.

Person Specification

  • Essential qualifications: Degree or diploma in Occupational Therapy, Current Health Care Professions Council registration as an Occupational Therapist, Advanced communications skills training or be prepared to work towards this, Evidence of Continuing Education, Car driver with current licence and access to a vehicle for work purposes.
