Berkshire Healthcare NHS Foundation Trust is seeking an experienced Registered Mental Health Nurse to join our Community Mental Health Team (CMHT) in Maidenhead. As a Mental Health Practitioner, you will be responsible for providing high-quality care and treatment to older adults with mental health conditions and co-existing physical health concerns.
Main Responsibilities- Conduct comprehensive assessments in patients' homes, taking into account their physical, psychological, social, and spiritual needs.
- Develop and implement care plans in collaboration with service users, families, and carers.
- Monitor and evaluate care plans, making adjustments as necessary.
- Manage a caseload, working collaboratively with other disciplines from statutory and voluntary sectors.
- Respond to acute psychiatric problems and take new referrals.
- Participate in service development, contributing to projects aimed at improving and maintaining quality of care.
- Registered Mental Health Nurse or equivalent qualification.
- Two years post-qualification experience.
- Ability to drive and have a car for use at work.
