Key Responsibilities of the Lead Care Facilitator
- Oversee the daily supervision of residents and staff, reporting to the Home Manager during their absence.
- Encourage effective teamwork by balancing individual skills, strengths, and areas for improvement.
- Lead shifts to ensure that staff provide person-centered care and fulfill the specific needs outlined in individual care plans.
- Assist the Home Manager in maintaining the efficient operation, organization, and management of the facility.
- Support the Home Manager in ensuring that each resident has a current, comprehensive, and tailored program of relevant, age-appropriate activities both in-house and in the community.
- Facilitate positive communication among staff, residents, and their families or representatives.
Work Schedule
Lead Care Facilitators are expected to work early, late, and weekend shifts as per a scheduled rota. This role supports a 24-hour service, requiring flexibility to work on Bank Holidays and nights as needed.
