Cornerstone Community Care is seeking a dedicated and compassionate Support Practitioner to join our team in Irvine. As a member of our small, dedicated team, you will provide 1-2-1 support to an individual with learning disabilities in their own home. Your main aim will be to support their day-to-day personal needs, including meal planning, cooking, housework, personal care, and medication. You will also be responsible for accompanying them on outings and engaging in activities that promote their independence and well-being.

Key Responsibilities:

• Provide high-quality support to individuals with learning disabilities in their own homes
• Assist with daily living tasks, such as meal preparation, personal care, and housework
• Accompany individuals on outings and engage in activities that promote their independence and well-being
• Maintain accurate records of support provided and individual's progress

Requirements:

• A caring and compassionate attitude
• Good communication and organisational skills
• Ability to work well in a team
• Positive attitude with a willingness to learn
• Confidence in the use of computers
