We are seeking a highly skilled and experienced professional to join our Discharge Team as a Discharge Coordinator. The successful candidate will work closely with our multidisciplinary team to support and promote patient flow, taking a lead on complex discharge planning and working towards a Home First Agenda.

Key Responsibilities

  • Support and promote patient flow across inpatient, emergency department, and community settings
  • Take a lead on complex discharge planning, working closely with the multidisciplinary team
  • Work towards a Home First Agenda, supporting early hospital discharge and admission avoidance
  • Provide leadership for junior members of the Discharge Team

Person Specification

We are looking for a candidate with:

  • NMC Registered or equivalent professional qualification in nursing, social work, therapy, or management in care services
  • Experience of working in complex discharge
  • Evidence of recent academic and professional studies
