About the Role

We are seeking a highly skilled and experienced Discharge Coordinator to join our team at Harrogate and District NHS Foundation Trust. As a Discharge Coordinator, you will play a critical role in supporting the discharge of patients from our hospital, working closely with our multidisciplinary team to ensure a smooth transition to community care.

Main Responsibilities

  • Lead on complex discharge planning, working closely with the multidisciplinary team to identify and address patient needs.
  • Develop and implement discharge plans, ensuring that patients receive the necessary support and care to enable them to return home safely.
  • Collaborate with healthcare professionals, including doctors, nurses, and therapists, to ensure that patients receive comprehensive care.
  • Provide leadership and guidance to junior members of the discharge team, promoting a culture of excellence and continuous improvement.
  • Work closely with our community partners to ensure seamless transitions of care and to identify opportunities for improvement.

Person Specification

  • NMC Registered or equivalent professional qualification in nursing, social work, therapy, or management in care services.
  • Experience of working in complex discharge, with a strong understanding of the needs of patients with frailty or complex care needs.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to work autonomously, with a high degree of independence and initiative.
  • Strong problem-solving and analytical skills, with the ability to think critically and creatively.
