About the Role

We are seeking a highly motivated and enthusiastic Assistant Practitioner to join our Post Covid Assessment Service team in Battersea Studios, SW8. As an Assistant Practitioner, you will work closely with a multidisciplinary team to deliver holistic and coordinated care to patients with post-covid-19 syndrome.

Main Responsibilities
  • Support community patients with post-covid-19 syndrome
  • Work alongside a multidisciplinary team to deliver coordinated care
  • Help coordinate care and case manage selected patients
  • Provide specialist treatment and support
Requirements
  • Good level of general education (GCSE or NVQ in Care equivalent)
  • Basic competency in assessment and equipment provision
  • Evidence of continuous development
  • Evidence of relevant Post Covid Training
  • Previous experience of directly working with clients within a health background
  • Previous NHS experience
  • Good verbal and written communication skills
  • Ability to work collaboratively within a multidisciplinary team
  • Ability to work independently without the supervision of the Allied Health Professional
  • Ability to liaise with others and report on client progress
  • Knowledge of Post Covid and any skills working in that sector
  • Community lone working experience
