About the Role

We are seeking a highly skilled and compassionate Social Worker to join our well-established therapies team at Priory Hospital Bristol.

As a key member of our multidisciplinary team, you will work closely with service users, their families, and other healthcare professionals to deliver high-quality care and support.

Your primary responsibility will be to coordinate and manage the mandatory processes and procedures to ensure the unit's compliance with current legislation and statutory regulation.

Key Responsibilities:
  • Develop and implement person-centred care plans that promote empowerment, equality, and social inclusion.
  • Work collaboratively with the multidisciplinary team to deliver prevention and early intervention strategies.
  • Undertake assessments, care plans, and tribunal reports to determine eligibility and support access to services under relevant social care frameworks.
  • Identify and address social exclusions, their causes, and effects on mental wellbeing and mental health.
  • Support service users to resolve their own problems and make sustainable support arrangements, including social care and health resources.
  • Promote carers' rights and access to assessments and resources.
  • Enable service users to be fully involved in their care and treatment, including planning, aftercare, and community support.
