Job Summary

The Corner Surgery is seeking a skilled Practice Nurse to join our team. As a primary care specialist, you will be responsible for delivering high-quality patient care and contributing to the overall success of our practice.

Main Responsibilities
  • Oversee the provision of chronic disease identification, monitoring, and management, ensuring patient assessment and treatment align with national and local guidelines.
  • Review and recommend changes to patient medications for chronic diseases, providing advice on dosages and side effects to enhance compliance.
  • Assess individual patient needs, ensuring healthcare is appropriate, evidence-based, and in accordance with the national GP contract and our Local Quality Contract (LQC).
  • Identify, manage, and support patients at risk of developing long-term conditions, implementing health promotion programs to prevent adverse clinical outcomes.
  • Develop, implement, and embed an effective care management program for frail and housebound patients, including home visits.
  • Perform practical investigations, including phlebotomy, processing of patient specimens, cervical cytology screening, and electrocardiograms.
  • Implement and administer national vaccination programs for adults and children, coordinating travel medication services.
  • Maintain accurate medical records, ensuring appropriate clinical templates and SNOMED-CT codes are used effectively.
  • Maintain the Practice Nurse and treatment rooms, ordering, stocking, and rotating items as required, ensuring general tidiness and cleanliness.
  • Recognize medical emergencies, supporting the team in their management, performing cardio-pulmonary resuscitation, and managing anaphylaxis in accordance with UK guidelines.
  • Lead on Infection Prevention and Control (IPC), working to the IPC policy, supporting the implementation of audits, training, and evidence for compliance to the Practice Manager.
  • Ensure adherence to practice policies and procedures at all times, including safeguarding, data protection, and chaperone policies.

  • Essential:
    • Working in a primary care environment.
    • Working autonomously.
    • Infection Prevention and Control (IPC) measures.
    • Clinical knowledge and skills:
      • Phlebotomy.
      • Processing of patient specimens.
      • Electrocardiograms.
      • Patient Group Directions.
      • Good interpersonal and communication skills.
      • Accurate medical record keeping.
      • Effective time management.
      • Competent in the use of MS Office software.
  • Desirable:
    • Working as a practice nurse or community nurse.
    • Chronic disease management.
    • Childhood immunization & cervical smears.
    • Managing frail and housebound patients.
    • Clinical knowledge and skills:
      • Chronic disease management.
      • Cervical cytology screening.
      • Vaccinations.
      • National and local GP surgery performance indicators.
      • Awareness of national and local health priorities.
      • Problem solving and analysis.
      • Clinical audit and risk management.
      • Competent in the use of EMIS Web software.
  • Other Requirements:
    • Flexibility to work outside of core office hours for the delivery of flu clinics.
