Position Overview: We are seeking a dedicated and proactive Community Care Support Specialist to join the Intermediate Care & Allocations Team within Adult Social Care at Lancashire County Council.
The Intermediate Care Allocation Team comprises a group of social care experts collaborating with hospitals and community services. Our mission is to facilitate safe and efficient hospital discharges and conduct comprehensive social care evaluations. We strive to prevent unnecessary hospital admissions by promptly assessing the care needs of our service users. As our new Community Care Support Specialist, you will engage with various services to ensure optimal outcomes for individuals both in hospital and at home, focusing on assessing the needs of service users, their caregivers, and necessary equipment.
This role operates in a dynamic environment that demands quick thinking and diligence, with the goal of enhancing the lives of our service users through innovative and evidence-based practices. Key Responsibilities:

You will cover multiple locations, so a proactive attitude is necessary, as the workload can be unpredictable, requiring resilience and adaptability.
Collaboration: While working independently is important, you will also collaborate with various Health and Social Care professionals. This established team values multi-disciplinary cooperation, and your contributions will be appreciated as you work closely with general practitioners, hospital wards, relatives, and community health services.
