We are seeking a motivated and experienced professional to join our Integrated Discharge Team as a Social Care Discharge Coordinator.
Key Responsibilities:- Triage and allocate referrals to appropriate workers to facilitate safe and timely discharges for individuals on a hospital discharge pathway.
- Supervise and provide complex case management advice to a team of Registered Practitioner Case Officers.
- Collect and maintain accurate data, and escalate any issues affecting the team's functionality to the Senior Practitioner and Team Manager.
The Integrated Discharge Team operates a discharge to assess model of care, which involves patients having their assessments outside of the acute hospital environment. As a Social Care Discharge Coordinator, you will support clients as they move through and beyond this pathway.
Adult Social Care and Health operates within modernised office processes and performance management systems. The service is increasingly accessible to the public outside of traditional office hours, with staff and managers working on a rota basis to ensure service needs are met.
You will have opportunities for training and development, and will receive the support you need and recognition for your good work.
This is an exciting time to join us, and informal visits prior to interview are welcomed from individuals who share our values and are ready to be part of our journey.
This role is subject to holding a full UK Driving Licence and an Enhanced Disclosure application to the Disclosure and Barring service, as well as registration as a qualified Social Worker with Social Work England.
