Role Overview

We are seeking a motivated and experienced professional to join our Integrated Discharge Team as a Social Care Discharge Coordinator.

As a key member of our team, you will work in partnership with other agencies and professionals to support the discharge pathway for clients moving through the acute hospital setting.

Your responsibilities will include triaging and allocating referrals, supervising and providing complex case management advice, and collecting accurate data.

You will also be responsible for escalating any issues affecting the team's functionality to the Senior Practitioner and Team Manager.

The Integrated Discharge Team operates a discharge to assess model of care, and you will support clients as they move through and beyond this pathway.

Adult Social Care and Health operates within modernised office processes and performance management systems, and you will be expected to work within these systems.

Alongside opportunities for training and development, you will receive the support you need and recognition for your good work.

This role is subject to holding a full UK Driving Licence and registration as a qualified Social Worker with Social Work England.
