Advanced Clinical Practitioner PCN Specialist

We are seeking an experienced Advanced Clinical Practitioner to join our team at Rural Warwickshire North PCN. As a key member of our Multi-Disciplinary Team, you will play a vital role in delivering high-quality patient care and contributing to the development of our Primary Care Network.
Key Responsibilities- Provide autonomous clinical decision-making and care to patients with undifferentiated and undiagnosed conditions.
- Work collaboratively with our digital team to implement and evaluate IT solutions that enhance patient care.
- Develop and maintain effective working relationships with key stakeholders, including GPs, consultants, and social services.
- Contribute to the development and implementation of PCN protocols and guidelines.
- Assist with the delivery of training and education to colleagues and other healthcare professionals.
- Hold a degree-level education and be registered with the NMC.
- Have a Master's degree in Advanced Practice or equivalent.
- Be an independent prescriber.
- Hold a CPD portfolio demonstrating ongoing learning and development.
- For Advanced Clinical Practitioner Nurses, have graduated from a Centre for Advancing Practice accredited MSc advanced practice programme or completed the Centre's ePortfolio (supported) Route.
- Teaching, assessment, and mentoring qualification.
- Leadership course.
- Experience working with frailty or similar community services.
- Understanding of relevant national strategy and policy.
- Ability to work in various locations throughout the speciality area.
- Driving license to travel to care homes and other locations across the PCN.
