We are seeking a dedicated Health and Social Care Support Navigator with a strong background in assisting vulnerable individuals. The ideal candidate will possess a comprehensive understanding of various sectors including health, financial assistance, social services, housing, legal systems, mental health, criminal justice, and substance dependency.
This role, internally referred to as a Specialist Care Navigator, involves working closely with individuals in correctional facilities and those on probation within the community.
Compensation:
£25,147 - £27,596 per annum, along with an array of excellent benefits.

Key Requirements:
Applicants must be at least 18 years old due to the nature of the work and must hold a valid UK Driving Licence with access to personal transportation. The position will require an Enhanced DBS check with Adults barred and Home Office Prison Clearance.
Job Responsibilities:
As a Health and Social Care Support Navigator, your responsibilities will include:
- Delivering specialized health care navigation services to individuals transitioning from North East Prisons, providing support both before and after their release.
- Collaborating with various agencies to enhance health outcomes and minimize reoffending rates.
- Conducting initial screenings and care mapping assessments for each service user, utilizing service documentation to plan their care and release.
- Creating customized support models for clients through diverse modalities and intensities to encourage independent self-management and engagement with both statutory and non-statutory services.
- Facilitating health promotion initiatives and organizing health-related events in partnership with colleagues.
- Maintaining detailed, accurate, and timely records for each patient.
- Employing a coaching framework to empower individuals in managing their release and associated healthcare needs independently.
Candidate Profile:
The successful candidate will possess:
- A minimum of NVQ Level 3 or equivalent qualifications relevant to the role, such as Health and Social Care or Mental Health.
- Experience in supporting vulnerable individuals to identify, establish, and achieve their personal goals through individual and group interventions.
- A solid understanding of health, financial assistance, social services, housing, legal systems, mental health, criminal justice, and substance dependency.
- A track record of providing structured, time-limited support to individuals.
- A passion for delivering high-quality care to individuals exiting the criminal justice system.
- Patience, calmness, and respectfulness in all interactions.
- A valid UK driving license and access to personal transportation for work-related duties.
