Salary: £20,316 to £23,316
Hours: 37.5 hours (Standard working hours are Monday to Friday, with flexibility required for evenings and weekends to accommodate service user needs)
Benefits: 30 days of Annual Leave (increasing to 35 days with length of service) + Bank Holidays, Company Pension, Life Assurance x2 & Employee Discount Scheme
Base Location: Mansfield, with a hybrid working model of 2 days per week in the office (Please note this is a community role, requiring travel to various locations)

Polaris Community is a well-established provider of children's services, dedicated to delivering innovative and outcome-focused support that adds real value and fosters brighter futures for children, young people, and families.
Our services span across Nottingham, Nottinghamshire, and Derby, providing intensive assistance to children and young people transitioning from residential care to fostering placements, stabilizing existing placements, supporting reunification efforts, and preventing children from entering care.
Requirements
This is a Community Role, necessitating travel to various locations.
- Experience in working with children and families in home visiting roles or delivering parenting programs.
- Ability to collaborate effectively within a professional network (including foster carers, social workers, and educational institutions) to advocate for family needs and preferences.
- Willingness to engage in ongoing learning and development to enhance your capabilities as a Family Support Practitioner and deliver evidence-based therapeutic programs.
- Residence in the Nottingham, Nottinghamshire, or Derby area to facilitate community visits.
- A full driving license with unrestricted access to your own vehicle.
Responsibilities
- Assist families receiving services under the designated support programs.
- Implement evidence-based therapeutic programs for parents and caregivers in the community to promote placement stability.
- Listen and respond with sensitivity to children, young people, and their families to facilitate positive change.
- Maintain regular contact with children, young people, and/or their families through face-to-face and telephone interactions.
- Ensure that the services provided uphold principles of equality, diversity, and individual rights.
- Deliver services in accordance with the established operational framework.
- Collaborate with other professionals and agencies to achieve the best outcomes for children, young people, and their families, ensuring a cohesive approach.
