The Patient Care Coordinator plays a vital role in orchestrating and enhancing patient-centered rehabilitation and discharge processes.

Primary Responsibilities:

Your role will involve ensuring the continuity and quality of care by collaborating with patients, caregivers, and multidisciplinary teams to plan and execute safe and timely rehabilitation and discharge, while also supporting the clinical staff.

You will be an effective communicator, capable of managing and prioritizing your workload, and demonstrating proficient IT skills.

Qualifications & Training:

Essential:

  • Foundation degree in Health and Social Care or equivalent knowledge, or Vocational level 3 education or equivalent skills, along with Level 2 numeracy and literacy.

Desirable:

  • Qualifications in Mental Health, Dementia, or End of Life care at Level 2/3.

Experience & Knowledge:

Essential:

  • Experience in an acute clinical setting.
  • Experience in multi-agency collaboration.
  • Experience in multidisciplinary teamwork.
  • Familiarity with IT systems.
  • Ability to demonstrate behaviors aligned with organizational values.
  • Proficient in communicating information in English, maintaining clear and accurate records.
  • Capable of presenting information clearly and logically, both in writing and verbally.
  • Able to work independently and prioritize your own caseload.

Desirable:

  • Awareness of local and national agendas within the specialty.
  • Knowledge of government initiatives regarding delayed discharge and social care.
  • Experience in patient discharge planning.
  • Customer service experience, particularly in handling complaints.
