About the Role

We are seeking an experienced Paramedic to join our team at The Chislehurst Partnership, a friendly and high-achieving suburban partnership in the London Borough of Bromley. As a Paramedic, you will play a vital role in delivering high-quality healthcare services to our patients, working closely with our multi-disciplinary medical team to provide safe, effective, and appropriate treatment.

Key Responsibilities
  • Assess and triage patients, including same-day triage, and provide definitive treatment or make necessary referrals to other members of the primary care team.
  • Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to the practice's social prescribing service and other community or voluntary services.
  • Perform specialist health checks and reviews within your scope of practice, interpret ECGs, perform investigatory procedures, and undertake the collection of pathological specimens as required.
  • Support the delivery of anticipatory care plans and lead certain services, such as monitoring blood pressure and diabetes risk in elderly patients.
  • Provide an alternative model to urgent and same-day GP home visits for the partnership.
  • Maintain accurate health records and ensure accurate completion of all necessary documentation associated with patient healthcare and registration with the practice.
  • Contribute to quality improvements and communicate effectively with patients and, where appropriate, family members regarding their physical health needs, results, findings, and treatment choices.
Requirements
  • Three years' experience as a Band 6 (or equivalent) paramedic.
  • Evidence of service development and experience with electronic clinical recording systems.
  • Experience working in a primary or community care setting.
  • Ability to assess and manage patient risk effectively and safely.
  • Able to analyze data and information for patient care plans.
  • Knowledge of audit principles and evidence of participation in audit/quality improvement projects.
  • Registered with the Health and Care Professions Council.
