Position Overview

We are seeking a dedicated individual to become a part of our team, working 30 hours per week, including both daytime and overnight shifts, across three distinct support services.

Service Areas

a) Independent Living Support: Assist an individual in achieving independence within the community while maintaining their living arrangements. Responsibilities include aiding with daily living tasks such as grocery shopping, home upkeep, managing health appointments, financial guidance, and facilitating social activities to foster friendships and skill development.

b) Community Engagement Service: This service provides assistance to individuals throughout the region, helping them access recreational activities, complete shopping tasks, and attend health-related appointments.

c) Community Support Initiative: In this role, you will support three young women who exhibit challenging behaviors, ensuring they receive the care and attention they need.

Core Responsibilities

As a Support and Care Facilitator, your primary goal will be to deliver exceptional support that significantly enhances the quality of life for those you assist, enabling them to lead more comfortable, dignified, and fulfilling lives.

  • Promoting Independence: Assist individuals with budgeting, bill payments, household chores, appointment organization, and preparing nutritious meals.
  • Effective Communication: Build strong relationships and help individuals understand their surroundings while fostering positive interactions.
  • Community Inclusion: Encourage participation in local events and activities to develop life skills and promote individuality.
  • Personal Care Assistance: Provide support primarily through prompting, helping individuals prepare for their day and assisting with personal hygiene tasks.
