We are seeking a highly motivated and experienced individual to join our team as a Social Prescribing Link Worker - Community Health Specialist. This is an exciting opportunity to work with our clients and help them take control of their health and wellbeing.
Key Responsibilities- Take referrals from a wide range of agencies, including GP practices, pharmacies, and social care services, to provide personalized support to individuals and their families.
- Develop trusting relationships with clients, providing non-judgmental support and respecting their diversity and lifestyle choices.
- Co-produce a personalized wellbeing plan with clients, focusing on their priorities and interests, to improve their health and wellbeing.
- Work with clients to identify the wider issues that impact on their health and wellbeing, such as debt, poor housing, and unemployment.
- Help clients maintain or regain independence through the development of everyday living skills, adaptations, and enablement approaches.
- Work with local agencies to raise awareness of social prescribing and encourage referrals.
- Provide regular updates to referral agencies about the development of the service and offer training on how to access information to encourage appropriate referrals.
- GCSE/NVQ level 2/3 or equivalent level of training and experience.
- Understanding of basic health and social care terminology.
- Proven ability to recognize and manage risk.
- Good interpersonal skills and ability to communicate confidently with staff at all levels.
- Ability to work well across teams and prioritize tasks effectively.
- IT skills and experience in the use of Microsoft Office programs.
- Familiarity with IT systems currently in use in health and social care settings.
- Experience in working directly in a community development context, adult health and social care, learning support, or public health/health improvement.
- Experience in supporting people with their mental health, either in a paid, unpaid, or informal capacity.
- Experience in working with the VCSE sector, including with volunteers and small community groups.
