The Royal Orthopaedic Hospital is seeking a skilled and compassionate Community Support Specialist to join our team. As a Community Support Specialist, you will play a vital role in supporting individuals with mental health needs in our community.
Main Responsibilities- Provide support and guidance to individuals with mental health needs, promoting independence and well-being.
- Work collaboratively with our multidisciplinary team to develop and implement personalized care plans.
- Conduct regular assessments and reviews to ensure the effectiveness of care plans and make adjustments as needed.
- Provide education and support to individuals and their families on mental health conditions, treatment options, and coping strategies.
- Collaborate with community resources and services to ensure seamless care coordination.
- Diploma in Health and Social Care level 2 or equivalent.
- Care Certificate.
- Key skills in Literacy and Numeracy level 2.
- Successful completion of Trust's full HCA Framework within 6-month probationary period.
- Previous experience as a Band 2/3 HCA or equivalent in a care environment.
- Previous experience of working with people with mental/social health needs or learning disabilities.
- Person-centred care.
- Awareness of legal frameworks including Mental Health legislation.
- Knowledge of Microsoft Office applications.
- Committed to continual quality and service improvement.
- Ability to travel independently in accordance with Trust policies and service need.
