Job Title: Social Worker - Pathways Leaving Care Team

We are seeking an experienced Social Worker to join our Pathways Leaving Care Team in Middlesbrough. As a Social Worker, you will work closely with care leavers, helping them to navigate the challenges of independence and achieve their full potential.

Key Responsibilities:
  • Manage a caseload of young people leaving care, ensuring their needs are assessed and addressed.
  • Develop and implement Pathway Plans that promote independence, well-being, and positive outcomes.
  • Support care leavers in accessing education, employment, housing, and other essential services.
  • Ensure compliance with relevant legislation, policies, and procedures related to leaving care and child protection.
Requirements:
  • Qualified Social Worker registered with Social Work England.
  • Significant experience in a social work role, particularly working with care leavers or young people in care.
  • Excellent communication and interpersonal skills, with the ability to build effective relationships with young people, families, and professionals.
  • Knowledge of relevant legislation, policies, and procedures related to leaving care and child protection.
