As a Community Health Specialist, you will play a vital role in developing and managing all aspects of our Continence Advisory Service to ensure high-quality delivery, patient satisfaction, and continuous improvements in patient-reported outcomes.

Key Responsibilities
  • Support the Continence Advisory Service Team Lead in developing and managing the service
  • Develop and implement evidence-based continence care practices
  • Collaborate with healthcare professionals to ensure seamless care delivery
  • Provide education and training to patients and healthcare staff
  • Contribute to the development of policies and procedures
Requirements
  • Registered Adult Nurse with a minimum of 3 years post-registration experience
  • Knowledge of evidence-based continence care practices
  • Community nursing experience
  • Basic IT skills, including working knowledge of SystmOne
  • Ability to work as part of a team and provide excellent patient care
