About the Role

We are seeking a highly motivated and empathetic individual to join our Social Prescribing Team as a Social Prescribing Link Worker. In this role, you will work closely with patients to identify their needs and connect them with community resources to improve their health and wellbeing.

Key Responsibilities
  • Provide one-to-one support to patients to help them identify their goals and develop personalized care plans.
  • Work collaboratively with local healthcare providers, voluntary sector organizations, and community groups to promote health and wellbeing services.
  • Establish strong relationships with patients, healthcare professionals, and community stakeholders to ensure effective communication and coordination of care.
  • Conduct needs assessments and develop care plans that address the physical, emotional, and social needs of patients.
  • Monitor patient progress, make adjustments to care plans as needed, and provide ongoing support to ensure optimal health outcomes.

  • High degree of empathy, understanding, honesty, and integrity.
