About the Role

We are seeking a highly motivated and empathetic Link Worker to join our Social Prescribing Team at York CVS. As a key member of our team, you will work closely with individuals to identify their needs and connect them to community resources, promoting health and wellbeing, and reducing demand on services.

Key Responsibilities

  • Provide one-to-one support to clients, exploring options to address non-clinical needs and improve their long-term health and wellbeing.
  • Work collaboratively with local VCSE, health and social care, and primary and secondary care organisations to support people with their health and wellbeing needs.
  • Establish links and build working relationships with primary and secondary care colleagues and other organisations to identify, build on, and support a range of services and community activities to promote health and wellbeing.
  • Keep accurate client records, collate qualitative and quantitative data, and feed in to quarterly and annual reporting.

  • High degree of empathy, understanding, honesty, and integrity.

  • Ability to work flexibly and effectively as part of a team.
  • Proactive approach to personal development and the updating of skills and knowledge.
  • Understanding of social prescribing and the value of VCSE sector services to support people with their physical and mental health.

Requirements

  • Significant experience of working with vulnerable people who may have multiple needs and complex lives.
  • Experience of person-centred service delivery in a community or clinical setting, advocacy, support work, counselling, or social work.
  • Experience of working collaboratively and in partnership with statutory and VCSE sector organisations.
  • Ability to work autonomously and to plan, prioritise work under pressure, and adapt to new models of working.
