Key Responsibilities:
- Fostering and implementing a person-centered support approach throughout the service
- Assisting individuals with various aspects of daily living in accordance with their support plans, which may include personal care and medication management
- Guiding individuals in acquiring independent living skills such as cooking and laundry
- Enabling access to community resources for social, educational, or employment purposes
- Upholding the rights and dignity of service users
- Actively participating in the development of support plans and risk assessments
- Ensuring effective communication within the team and attending regular meetings with team members and service users
- Professionally engaging with family members and other professionals.
