Job Summary

An exciting opportunity has arisen to join the cardiac rehabilitation team based within the community setting. This part-time post involves working across various sites, making driving an essential part of the role.

Main Responsibilities
  • Assess, plan, and evaluate individualized care with patients following a diagnosis of coronary heart disease.
  • Facilitate Heart manual, Revascularization Manual, and Angina plan.
  • Support families and carers.
  • Contribute to the development of local protocols and pathways based on evidence such as NICE and BACR.
  • Participate in delivering pathways of care in patients' homes and community venues through phase 2, 3, and 4 of cardiac rehabilitation.
  • Deliver primary preventative and secondary preventative strategies and advice to patients to support those ready to change their exercise lifestyle.
