Cardiac Rehabilitation Specialist Nurse

We are seeking a highly skilled and experienced Cardiac Rehabilitation Specialist Nurse to join our team at North Tees & Hartlepool NHS Foundation Trust. As a key member of our cardiac rehabilitation team, you will play a vital role in providing high-quality care to patients with coronary heart disease.
Main Responsibilities- Assess, plan, and evaluate individualized care for patients with coronary heart disease.
- Facilitate the development and implementation of personalized care plans, including the Heart Manual, Revascularization Manual, and Angina Plan.
- Provide education and support to patients, families, and carers to promote healthy lifestyle choices and disease management.
- Contribute to the development of local protocols and pathways based on evidence-based guidelines, such as NICE and BACR.
- Participate in delivering pathways of care in patients' homes and community venues through phase 2, 3, and 4 of cardiac rehabilitation.
- Deliver primary and secondary preventative strategies and advice to patients to support them in making lifestyle changes.
