We are seeking a dynamic and experienced Social Care Coordinator to join our Mental Health Social Care Team at Reading Borough Council. As a key member of our team, you will play a crucial role in promoting wellbeing and independence for individuals with complex mental health difficulties.
Key Responsibilities:- Collaborate with Social Care Coordinators, Social Workers, Occupational Therapists, and professionals from the voluntary and community sector to provide comprehensive support services.
- Work closely with individuals to implement interventions that reduce risks and support positive outcomes, using a strengths-based approach.
- Hold a caseload of individuals with mental health difficulties and work independently with support and supervision from an experienced Social Worker.
- Develop and maintain a good understanding of risk management, flexible and able to analyze varied information or situations to produce solutions.
- Experience working with people with mental health difficulties and/or complex needs.
- Experience working within the Health, Adult Social Care or voluntary sector (not essential).
- Ability to work within the legal framework provided by statute, guidance, policies, and procedures.
