About the Role:

We are seeking a highly motivated and experienced Senior Care Coordinator to join our Reading Supported Living Service. As a Senior Care Coordinator, you will play a key role in ensuring the smooth operation of our service, providing excellent care and support to our residents, and working closely with our office team to achieve our goals.

Key Responsibilities:

  • Lead a team of Support Workers to deliver high-quality care and support to our residents
  • Ensure that all necessary paperwork, including Support Plans, Risk Assessments, and MAR charts, is completed and updated regularly
  • Work closely with our office team to ensure the smooth operation of our service
  • Support residents with direct care visits and monitoring checks, including medication competency and carer assessments
  • Work with clients to ensure they receive the best possible care and support from Radis

Requirements:

  • Minimum 1 year of experience working in the health and social care sector, preferably in a Supported Living or Mental Health setting
  • Ability to work a range of shift patterns, including alternate weekends
  • Access to a vehicle for travel between services within the Reading area
  • Currently hold or working towards a NVQ Level 3
  • Ability to work as part of a team and provide excellent care and support to our residents
