Job Summary

We are seeking a highly motivated and experienced professional to join our team as a Primary Care Transformation Lead. This is an exciting opportunity to work closely with our general practices, Primary Care Networks, GP federation, and wider health and care partnership across Greenwich.

Main Duties

The successful candidate will play a key role in the Greenwich Primary Care team, working closely with all team members to deliver high-quality and efficient services. Key responsibilities include:

  • Developing collaborative relationships with Greenwich general practices, Primary Care Networks, and partner organisations to support primary care transformation and sustainability.
  • Leading or supporting primary care improvement initiatives, service redesign, and procurement to deliver transformation in Greenwich and South East London.
  • Participating in patient, public, and carer engagement through various methods and analysis.
  • Staying up-to-date with health and care policy development and contributing to South East London and Greenwich strategy and planning.
  • Deputising for colleagues and leaders as required and representing Greenwich borough or South East London at external meetings and events.

  • Strong communication and relationship-building skills.

  • Good time management and prioritisation skills.
  • Confidence in using Microsoft Office applications.
