Job Summary

The Information Governance Team, within the Finance and Resources department of Leeds Community Healthcare NHS Trust, is responsible for providing support to the Data Protection Officer, internal information governance arrangements, and records management. These include the development and maintenance of policies and procedures, information sharing arrangements, the management of security incidents, information governance training, data security and protection toolkit compliance, information security, and support to the Caldicott Guardian and Senior Information Risk Owner (SIRO).

Main Duties and Responsibilities
  • Ensure the delivery of an effective information governance support service.
  • Support the Information Governance Manager and Deputy Data Protection Officer in working with multi-disciplinary staff and developing, implementing, and maintaining robust information governance programs to help ensure the trust's compliance with its statutory and regulatory obligations, including the Data Security and Protection (DSPT) Toolkit.
  • Assist the IG Manager and Deputy Data Protection Officer in the day-to-day management and implementation of the trust's information governance compliance programs and the risk and projects management framework.
  • Provide general information governance support, advice, and guidance to all staff at different levels.
  • Develop and maintain good communications and working relationships with relevant stakeholders within and outside the organization.
Key Responsibilities
  • Act as a point of contact for queries relating to the trust's information governance policies and procedures, handling telephone calls and enquiries from trust staff, patients, the public, and staff of both NHS and non-NHS organizations.
  • Support the IG Manager and Deputy Data Protection Officer with the coordination of the information requests received under the Freedom of Information Act (FOI) 2000 and the Data Protection Act (DPA) 2018.
  • Provide support to business areas with queries on FOI/DPA 2018 and information rights queries.
  • Provide information rights/subject access procedure training to teams and individuals when requested.
  • Provide and collate compliance information for the trust's IG Group.
  • Liaise with trust staff and staff from partner organizations to obtain information required for disclosure purposes.
  • Assist the Information Governance Department in motivating and negotiating with staff to achieve required outcomes for mandatory and statutory deadlines in relation to information governance.
  • Monitor the progression with the uptake of IG training and produce data on the trust's compliance with IG training.
  • Support the development and implementation of training packages.
  • Presentation of the IG slot at the trust induction.
  • Assist the Head of Information Governance in the development of communication channels and the dissemination of information.
  • Develop an in-depth knowledge of the information governance policies and procedures and support the IG Manager and Deputy Data Protection Officer with their implementation.
  • Support the breach management process and provide administrative support to the IG Manager and Deputy Data Protection Officer.
  • Monitor and update incidents via DATIX, assess and record the breach severity scoring, and collate incident management information to support quarterly reports.
  • Monitor the DATIX system to ensure that all IG-related incidents have been reviewed, remedial actions implemented, and the incident is closed in a timely manner.
  • Assist with the development and dissemination of relevant lessons learned via internal communication channels.
  • Develop an in-depth knowledge of the Freedom of Information Act 2000 and the Data Protection Act 2018.
  • Use initiative to resolve more complex information governance issues where there may be a range of options to explore, request under the Freedom of Information Act 2000.
  • Identify areas that would impact on the delivery of statutory and mandatory information governance requirements, identifying potential solutions where possible.
  • Exercise own judgment when dealing with enquiries via telephone and in person.
  • Assist the Information Governance Officer and Head of Information Governance as required.
  • Undertake small projects under the direction of the Head of Information Governance.
  • Plan and organize audit activity within the Information Governance department and across multiple sites, in line with the requirements of the Data Security and Protection Toolkit (DSPT).
  • Provide general advice to departments/teams on records management and retention, undertake and support records management audits.
  • Support IAOs and IAAs with recording, updating, and reviewing identified information assets and associated data flows.
  • Prioritize and plan own workload, ensuring specific deadlines are met.
  • Collect and collate routine data, as requested, to assist with the compilation and preparation of written reports.
  • Ensure that the trust provides information in line with the required legal timescales.
  • Provide non-clinical information governance advice.
  • Liaise with patients/representatives to support the Information Governance Manager and Deputy Data Protection Officer with providing data protection advice and guidance to the public.
  • Implement policies for own work area and propose changes to procedures and/or working practices in own area.
  • Demonstrate information governance duties to new or less experienced staff.
  • Produce a range of documents, including reports, letters, presentation slides, using appropriate software packages Word, Excel, and PowerPoint.
  • Maintain the Caldicott and Data Protection and Freedom of Information logs and the Information Governance risk register as required, plus additional spreadsheets/databases as required.
  • Be responsible for the maintenance of information governance systems, both manual and electronic, in accordance with trust policy.
  • Maintain the Data Protection Impact Assessment (DPIA) register and ensure approved assessments are uploaded to the trust publication scheme after being anonymized.
  • Monitor and manage the team email inboxes.
  • Maintain the trust privacy notice information on the trust website under the guidance of the IG Manager and Deputy Data Protection Officer.
  • Undertake surveys or audits as necessary to own role.
