Our adult social care teams encompass a range of services, including:

Learning Disability Support Team

This team conducts initial adult care assessments and reviews for vulnerable young adults with learning disabilities and their caregivers, collaborating with service users to develop care and support plans tailored to their needs. This role specifically focuses on young individuals transitioning into adult care.

Autism Support Team

The Autism Support Team is tasked with evaluating and assisting adults diagnosed with an Autistic Spectrum Condition (ASC), adhering to the Care Act 2014.

Social Care Assessment Unit

This unit provides short-term assistance to service users following hospital discharge, working closely with the reablement service as part of the discharge assessment model.

First Response Unit

This unit serves as the initial contact point for the public and professionals seeking assistance from Adult Social Care, whether they are new to the service or do not have an assigned worker.

To be considered for this role, you should possess:

  • A relevant qualification in social work
  • Understanding of the legal framework and regulations governing Adult services
  • Knowledge of applicable social work theories and practices
  • Proficiency in standard electronic systems
  • A genuine passion and commitment to supporting the most vulnerable individuals in our community
