We are seeking a highly skilled and compassionate Registered Nurse to join our team at East Coast Community Healthcare CIC. As a Registered Nurse in our Intermediate Care Centre, you will play a vital role in providing high-quality care to patients in our community.
Key Responsibilities- Contribute to the on-going assessment, provision, and monitoring of care in line with our service pathways and in collaboration with the multi-professional team.
- Monitor the condition and health needs of patients on a continual basis, working in partnership with the multi-professional team.
- Provide holistic and person-centered care and support for patients of all ages and in various settings.
- Act as a patient advocate, respecting patient confidentiality, privacy, and diverse cultural backgrounds and requirements.
- Participate in multidisciplinary team meetings and contribute to the development of care pathways for patients with complex healthcare needs.
- Work collaboratively with colleagues to deliver evidence-based, cost-effective care and promote a culture of continuous improvement.
- Contribute to the review of current policies, protocols, and guidelines, and the development of clinical pathways and outcomes for inpatients.
- Plan and organize complex clinical activities to ensure best practice is delivered to patients in your delegated area of responsibility.
- Current registration with the Nursing and Midwifery Council (NMC).
- Experience working as a Registered Nurse in a hospital setting.
- Excellent written and verbal communication skills.
- Ability to work on own initiative and autonomously.
- Willingness to work flexible hours and locations to accommodate the needs of the service.
- Ability to embrace our culture, values, and signature behaviors.
