Nurse Specialist

We are seeking a highly skilled and compassionate Registered Nurse to join our team at East Coast Community Healthcare CIC. As a Registered Nurse, you will be responsible for providing high-quality patient care in our Intermediate Care Centre, working closely with a multidisciplinary team to deliver rehabilitation and specialist palliative care services.
Main Responsibilities- Contribute to the ongoing assessment, planning, and implementation of patient care in line with service pathways and agreed treatment plans.
- Monitor patient condition and health needs, working in partnership with the multi-professional team to ensure safe and effective care.
- Act as a patient advocate, discussing treatment options with sensitivity and expertise, and respecting patient confidentiality and diverse cultural backgrounds.
- Participate in multidisciplinary team meetings, contributing to the development of care pathways and outcomes for patients.
- Take charge of the service and/or site in the absence of a Sister/Charge Nurse, leading the team and maintaining a safe environment.

Person Specification- Essential:
- Working as a Registered Nurse in a hospital setting.
- Desirable:
- Evidence of experience working within a community hospital setting.
- Evidence of experience working within a secondary care environment.
- Evidence of experience working to improve clinical practice.
- Evidence of experience working with a case management approach to care.
