We are seeking a skilled Community Support Worker to join our team at Tewv Brits. As a Community Support Worker, you will play a vital role in supporting individuals with mental health needs, providing a range of interventions to improve their lives and outcomes.
The successful candidate will have completed a Diploma in Health and Social Care Level 2 or equivalent, and will have Literacy and Numeracy level 2 or equivalent. They will also be required to complete the Trust Healthcare Assistant competencies within their probationary period.
Main Responsibilities- Undertake delegated activities within their level of competency and under the supervision of qualified staff, with due regard to risk assessment.
- Support the delivery of interventions to individuals, including graded exposure and anxiety management.
- Carry out physical health monitoring, including blood pressure, temperature, pulse, and obtaining blood and physical health checks, and discussing results with appropriate professionals.
- Support the development and implementation of care plans, providing appropriate feedback to clinicians and communicating outcomes.
- Keep accurate electronic records.
- Diploma in Health and Social Care level 2 or equivalent.
- Care Certificate.
- Key skills in Literacy and Numeracy level 2.
- Successful completion of Trust's full HCA Framework within 6-month probationary period.
- Previous experience as a Band 2/3 HCA or equivalent in a care environment.
- Previous experience of working with people with mental/social health needs or learning disabilities.
- Person-centred care.
- Awareness of legal frameworks, including Mental Health legislation.
- Knowledge of Microsoft Office applications.
- Able to work in accordance with the Staff Compact and Trust Values and Behaviours.
- Compassionate in meeting the needs of vulnerable people and their families and carers.
- Able to work flexibly and co-operatively as part of a team.
- Committed to continual quality and service improvement.
- Self-awareness and committed to continual personal development.
- Ability to travel independently in accordance with Trust policies and service need.
