We are seeking a highly skilled and experienced Primary Care Mental Health Triage Practitioner to join our team at the South Westminster Community Mental Health Hub. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care to adults with mental health issues.
Key Responsibilities- Conduct comprehensive psycho-social mental health assessments to establish care needs and develop personalized care plans.
- Act as a liaison between primary care and secondary care services, facilitating referrals and ensuring seamless transitions for patients.
- Provide short-term interventions for clients with complex and severe mental health issues, working collaboratively with GPs and other healthcare professionals.
- Support the safe discharge of patients from secondary care, ensuring effective continuing care and access to support services.
- Develop and maintain strong relationships with GPs, practice nurses, and other stakeholders to promote collaborative working and improve patient outcomes.
- Current professional qualification in mental health nursing or occupational therapy, with registration with the relevant professional body.
- Extensive post-registration experience in mental health, with a strong understanding of psychosis and its impact on service users and carers.
- Excellent communication and interpersonal skills, with the ability to build constructive relationships with clients, carers, and colleagues.
- Knowledge of current legislation and guidance applicable to mental health services, with a commitment to equal opportunities and diversity.
