Job SummaryWe are seeking a highly skilled and compassionate Healthcare Facilitator to join our team. As a key member of our healthcare team, you will play a vital role in ensuring the smooth delivery of patient care and services.
Key Responsibilities- Link between Clinicians and Clinical Correspondence: You will act as the primary point of contact for clinicians and clinical correspondence, ensuring that all documentation is accurately scanned, coded, and matched using our electronic patient record system.
- Front-line Interface between Patients and the Practice: You will provide exceptional patient communication, issuing prescriptions and responding to patient inquiries in a professional and courteous manner.
- Facilitate Preventative Healthcare: You will work closely with clinical and administrative colleagues to deliver preventative healthcare services to our patient population.
- Liaise with Patients and Care Facilitate Enquiries: You will provide support and guidance to patients and their families, ensuring that their healthcare needs are met and their concerns are addressed.
- Qualifications: GCSE grade A to C in English and Maths
- Experience: Essential experience working with members of the public, using IT, and possessing stellar communication and interpersonal skills. A team player with a dash of independence is also essential.
- Desirable: Experience working in Primary Care or a healthcare setting, and experience using a patient clinical system such as SystemOne.
- NHS Pension: We offer a competitive NHS pension scheme with a 14.38% employer contribution.
