Job Summary
We are seeking a skilled Discharge Planning Specialist to join our team at Connect2Hampshire. As a key member of our Hospital Discharge Team, you will play a vital role in supporting the turn-around of patients at the front door and working with the Single Point of Access at the hospital.
Key Responsibilities
- Collaborate with the acute team to ensure seamless patient care.
- Assess patients for discharge planning and develop effective care plans.
- Work closely with the Single Point of Access to ensure timely discharges.
- Provide expert social work support to patients and their families.
Requirements
- 3 years of post-qualifying experience in social work.
- Hold a social work degree.
- Proven experience of working with children and families in a social work setting.
- Knowledge of child protection legislation and understanding of child protection issues and substitute care.
