Job Summary

An exciting opportunity has emerged for an individual with exceptional organisational skills and self-motivation who relishes the challenge of joining an innovative Heart Failure team dedicated to delivering specialist care within the community.

Main Duties of the Job

The position involves home visiting to assist Heart Failure specialist nurses in delivering holistic and comprehensive care.

Duties include conducting ECGs, attaching ambulatory diagnostic devices, and recording basic measurements like height, weight, and blood pressure.

Additionally, the role entails drawing blood samples, both venous and capillary, for Point of Care Testing (POCT), aiding the nurses in managing heart failure symptoms and minimizing hospital admissions.

Person Specification

Qualifications and Professional Training

  • Nursing Associate Foundation Degree or equivalent training/experience
  • GCSE Grade A-C Maths and English or skills level 2 Maths and English or equivalent.
  • Phlebotomy and venepuncture competencies / training

Desirable

  • Registered Nursing Associate on NMC register

Experience and Knowledge

  • Understanding of the scope of the role of the Nursing Associate in context of the nursing and interdisciplinary team and the organisation, and how the role may contribute to service development
  • Understands and acts in line with NMC professional standards for practice contained within Standards of Proficiency for Nursing Associates.
  • Have a level of knowledge of heart failure inclusive of causes and treatment along with basic knowledge of a broad range of conditions experienced by patients in community and general practice settings.
  • An understanding of the presentations of multiple pathology, depression and anxiety states and frailty, predominantly in older people.
  • Knowledge of when to seek advice and escalate to the appropriate professional for expert help and advice
  • Understand the importance of following procedures and treatment plans
  • Experience of working in teams under appropriate supervision as part of a multi-disciplinary team
  • Insight into how to evaluate own strengths and development needs, seeking advice where appropriate
  • Experience of providing and receiving complex, sensitive information

Desirable

  • Experience of working within the community setting
  • Understand requirements for NMC professional Revalidation
