Position Overview

An exceptional opportunity is available for a qualified Pharmacist to join our team as a Clinical Pharmacy Specialist within a Primary Care Network (PCN).

This role is centered in practice, where the successful candidate will collaborate with multiple practices, concentrating on elderly patients experiencing frailty (multi-morbidity) and individuals with chronic conditions such as cardiovascular diseases, asthma/COPD, and diabetes. The ideal candidate will also assist practices in enhancing the management of atrial fibrillation through suitable anticoagulation, monitoring high-risk medications, and ensuring prescribing safety in accordance with the latest GP contract guidelines.

Key Responsibilities

The selected candidate will function as part of a multidisciplinary team in a patient-facing capacity, taking charge of chronic disease management within the practices and conducting clinical medication reviews to effectively manage patients with complex medication regimens. They will also serve as the primary resource for general practice staff regarding prescription and medication inquiries.

The Clinical Pharmacy Specialist will be expected to cultivate strong working relationships both internally and externally to uphold the high standards that the practice strives to achieve. Our mission is to implement meaningful changes collectively to improve the quality of care provided to our patients.

This organization aims to coordinate activities around patient groups requiring ongoing care while continuing to deliver standard GP services. We are committed to focusing on population health and supporting preventive initiatives, including vaccination programs.

Role Responsibilities

  • Conduct patient-facing clinics for long-term condition management.
  • Evaluate patients with single or multiple medical issues requiring medication optimization (e.g., COPD, asthma).
  • Review the necessity of each medication, monitoring requirements, and assist patients in optimizing their medication use.
  • Provide recommendations to Senior Pharmacists or GPs for medication improvements.
  • Perform clinical medication reviews with patients and provide recommendations to senior clinical pharmacists, nurses, or GPs on prescribing and monitoring.
  • Collaborate with care home staff to enhance the safety of medication ordering and administration.
  • Manage common/minor ailments within the scope of practice.
  • Offer telephone support for patients with medication-related queries.
  • Analyze and present medication data to highlight issues and support decision-making.
  • Engage in quality improvement initiatives and implement changes based on national guidelines.

Qualifications and Experience

  • Minimum of 2 years post-qualification experience.
  • In-depth therapeutic and clinical knowledge.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively both independently and as part of a team.
  • Masters degree in pharmacy (MPharm) or equivalent.
