  • Deliver a smooth transition of new clients from acute NHS services, liaising with treating medical, therapeutic, and care staff to ensure seamless care.
  • Act as a link between lawyers, public bodies, agencies, charities, NHS, and DWP to ensure effective communication and secure additional statutory or voluntary services where required.
  • Provide on-going support, signposting, coordination, or practical support to clients and their families, ensuring they are maximizing their statutory welfare entitlement.
  • Prepare and showcase our SRC service to NHS Trusts, charities, and outside agencies to promote our services and encourage referrals of new clients.

  • Knowledge of healthcare, rehabilitation, welfare policy, and advocacy services.
  • Understanding of relevant rehabilitation codes, Mental Capacity Act, and welfare benefits.

  • Experience of working within a Multi-Disciplinary Team (MDT) framework.
  • Confident to work autonomously and independently.
  • Full UK driving license and access to a car.

Our Benefits

  • 25 days' holiday as standard, plus bank holidays.
  • Generous and flexible pension schemes.
  • Volunteering days – two days of volunteering every year for a cause of your choice (fully paid).
  • Westfield Health membership, offering refunds on medical services alongside our Aviva Digital GP services.

We offer a wide range of well-being initiatives to encourage positive mental health both in and out of the workplace. We have been ranked in the Best Workplaces for Wellbeing for Large Organisations for 2024.

We are committed to being a diverse and inclusive workplace where our colleagues can flourish. Our responsible business programmes are fundamental to who we are and our purpose.
